I’ve been using Rememberthemilk. I am changing to nozbe. The google calendar implementation at RTM is awfull and you have to manage a lot of endlessly long list. Its a very overwhelming interface.

I still like OmniFocus, and use it every day. I admit being a little disappointed that they charge for iPhone, iPad and Mac independently, but I’m past that now. Anything specific you’d like to know?

Maybe have a look at Apollo as well: http://www.apollohq.com
It’s project and contact management, with cases&deals, timers, and a great calendar.
We worked really, really hard on its user interface.

I've been trying out Nirvana recently (Nirvanahq.com) and it's really really impressed me. As well as being pretty it seems to better drill down to the core principles that I ever found Nozbe did.

Transferring from one system to another can be a pain though. I'm just about moved over, and I'm a serial GTD app jumper. Hopefully I can finally rest with Nirvana.

I really think that writing lists by context has helped me tremendously. I am not quite as detailed as you yet, but at the very least, it has helped me from making 12 trips to the grocery store for forgotten items and calling back my doctor 3 times.
